Reminders for Parents:
Immunizations: Please make sure your child is receiving their immunizations and a copy of the record is given to the school nurse. Indiana law (Indiana code 20-8.1-7-10.1) provides that no student shall be permitted to attend school for more than twenty(20) calendar days beyond the date of his/her enrollment if immunizations are not up to date.
Medical Concerns: It is very important for the school nurse to be notified of any medical concerns of your child. Please do not hesitate to let me know of any changes and updates to your child medical concerns. If you have any questions or concerns feel free to contact me.
Medications: In order for students to have prescription medication given by the school nurse a form must be completed by the ordering physician as well as the parent/guardian. In addition, if a student has over- the- counter medication including cough drops a non-prescription formed must be filled out by a parent/guardian.
Female students: Please make sure your daughter bring in a change of clothing along with supplies (sanitary napkins/tampons) to keep in their locker. Also, sanitary napkins/tampons are available in most bathrooms for 5 cents and 25 cents in others.
