The goals of the Open Door Adolescent Health Center are to reduce student absenteeism, minimize the interruption of education for health reasons and to improve students' access to comprehensive health/mental health services and education.Enhancing Health Care
Open Door Adolescent Health Center is not meant to be an emergency clinic or primary care provider. Our staff makes every effort to work closely with a student's primary care provider and to make appropriate referrals for additional care whenever necessary.Registration ProcessParental consent is required before any student may receive services.Due to COVID-19 restrictions, the Open Door Adolescent Health Center will allow online registration for the 2020-2021 School Year only. Please follow the link below to complete the online form. Once the form is received, you will receive a telephone call to authorize registration. This form will not be valid without additional verbal authorization confirmed by ODAHC staff.All Students 18 years or older may register themselves in the program. Once the registration is complete, it will remain open until your student(s) graduate, unless you contact our office in writing to revoke permission.Note: All services are free of charge regardless of the students' or parents' status (economic/insurance) however, we do ask for their insurance information for statistical purposes and their social security numbers for identification purposes.
